Q:My member card is missing. What shall I do?

A:Each new member will receive a member card issued by this Club as per the membership agreement. Valid member card issued by this Club is the only ticket for a member to enjoy membership benefits when accessing to service at this Club.
　　
When the member card is missing or damaged, the members concerned shall immediately report it to the issuing club, and fill out a Card Missing Report. The card holder will be liable for the expenses under the account of the missing or damaged card until this Club receives the missing or damage notice formally. Once the card missing or damage fact is verified by this Club, the original member card will be cancelled and superseded by a new card, for which the member concerned needs to pay ￥10 as handling fee (which is subject to change from time to time).
